Make sure to check and double-check all the settings. I've had a number of weird sound issues with the game where settings just changed for no reason at all; anything from sound being set to mute, to volume being 0, to the game somehow blocking all other sound channels. This is the problem. When you start FFXIV, it detects the output device/Speaker and configures itself to use this. This isn't a problem if you are using a sound card like Creative's Sound Blaster.
