Give your thrall a good bleed weapon like daggers. If you're comfortable up close apply poison to your weapon. If you'll be shooting arrows mix in poison, oil, explode arrows. But make sure your thrall is attacking because, you're right, when you exclusively use ranged their health will randomly regenerate, so keep the DPS going.
